What do I need to bring to my appointment?
If this is your first appointment, please print out and complete the medical history or dental history form and HIPAA release, and bring them with you. Bring your current insurance cards and all medications you are taking with you to each appointment.

Bringing your medication to each appointment helps the doctor to know exactly what you are taking and if you will need any refills.

What if I need to get my medication refilled before my appointment?
We need a 24-hour notice on all prescription refills. If you need to have your prescription refilled sooner than that, you will be charged a fee, and you will need to pick up the doctor’s written prescription order at the health center.
