Most insurances and credit cards are accepted. Extended hours are offered. Walk-ins and Access patients are welcome. Discount and drug assistance programs are offered for the uninsured and underinsured.
Medical Director Dr. Stephen C. Love, MD oversees medical staff and operations. Certified Registered Nurse Practitioner Tamra L Hansford is certified in Women’s Health. Registered Nurse Kimberly Young coordinates the Medication Assistance Program and Sliding Fee Program.
Dental Director Dr. Ryan Pensyl oversees dental staff and operations. He also performs general dentistry and preventive dental care procedures, which includes restoration and extractions, root canals, crowns, bridges and dentures. Registered Dental Hygienist Lindsay Pfeiffer, assists with soft tissue management, cleanings and applying sealants (a protective covering on children's teeth).
The providers are on call evenings and weekends for emergencies (the answering service will contact them); and house calls can be arranged if needed. A dentist also is available most times for dental emergencies. When a dentist is not available, patients are referred to a hospital.
We are a health care team,
available to assist you in the way that is best for you.
New patients
Medical history, dental history and HIPAA permission forms can be downloaded here for your convenience. Please print these forms out, complete them, and bring them with you to your first appointment. Please also read the financial policy prior to coming in for your appointment.
Sliding Fee Discount
Because we are a Federally Qualified Health Center, we can offer a sliding fee discount program for those who qualify. Based on your family size and income, you may qualify for a discount of 60 to 90 percent of fees charged for medical services and up to 50 percent on covered dental and counseling services. Some minimal charges to the patient apply at each visit. This discount applies to almost all services, but there are a few exceptions. Insurance carriers are billed first; then the discount applies to any remaining balance.
If you are interested, print out the application now, complete it, and mail or bring it in prior to your appointment. You must provide proof of income to be considered. The information is confidential. Those who receive the discount are required to reapply one year afterward or whenever changes occur in income or family size.
Prescriptions
Prescription refills are issued during office hours only. If you require a refill of you prescription, please contact your pharmacy and ask them to fax the request to our office. If your prescription has expired, and you have not been seen in the office in the past six months, we will need to schedule an appointment for you to come in and be evaluated by a provider before renewing your prescription.
Telephone calls
Your calls are welcomed, and we will answer you the best we can. However, a telephone conversation cannot substitute for an appointment and examination. We may require that you come in and be evaluated by a provider.
Lab results
Lab tests that are processed at the health center include blood sugar, hemoglobin, urine pregnancy, urinalysis, rapid streps and cholesterol. More complicated tests are sent out to a laboratory. Some tests can take several days, and Pap smears may take up to 10 days to process.
Miscellaneous
Information will not be provided to a third party (attorney or insurance company) without your written authorization. Information is provided to family only with the patient’s permission. Please keep in mind that when a patient is hospitalized, we like to discuss the situation with a family member who then can forward information to other members of the family.
Conclusion
We value your confidence in our practice. We believe that your understanding of your health condition and active participation in your treatment are important parts of your overall care. Therefore, when a diagnosis is made or treatment is recommended, it will be thoroughly explained and discussed with you. If you have any questions, we will answer them as fully as we can. If you should have questions or concerns after leaving our office, please do not hesitate to contact us. |
